This page is for the convenience of players and parents to have the necessary tools and information for the participation in El Toro Girls Soccer. It is up to the players and parents to ensure that the coaching staff has all necessary and completed forms with signature turned in, before participation can begin.

 

Athlete Clearance:

To be eligible to play on a team in the winter season, the athlete must complete the Athlete Clearance process through the following site:

https://www.athleticclearance.com/.

 

Part of the process will include uploading a current Sports Screening form. The Sports Screening must be less than 1 year at the end of season ... so in other words, the form must have been completed after March 1, 2022.

 

Be sure to complete the first page of the below Sports Screening form and have your doctor complete page 2. The completed form will be turned in as part of the Athlete Clearance process.
